Aceclidine is a pupil‑preferential muscarinic cholinergic partial agonist. It mainly acts on muscarinic receptors of the iris sphincter muscle with minimal stimulation to ciliary muscle. Contraction of iris sphincter produces miosis and creates a physiological pinhole effect, which extends ocular depth‑of‑focus and improves near visual acuity without significant myopic shift or deterioration of distance vision. The marketed formulation is preservative‑free single‑dose ophthalmic solution. Onset of visual improvement occurs within approximately 30 minutes post‑instillation, and therapeutic effect can sustain up to 10 hours. It is developed as a non‑surgical pharmacotherapy for adult presbyopia. Historically, aceclidine has also been used for open‑angle glaucoma via intraocular pressure‑lowering miotic action.
⚠️ Safety brief note
Most common adverse reactions include ocular irritation, conjunctival hyperemia, transient blurred vision and headache. Avoid use in patients with hypersensitivity to aceclidine. Caution for patients with risk of retinal detachment. Not recommended for children.
