PZM21 is a landmark G protein-biased μ-opioid receptor (μOR) agonist discovered by structure-based virtual screening and rational lead optimization. Distinct from classic morphinan opioids, it selectively triggers Gᵢ protein signaling while showing minimal β‑arrestin‑2 recruitment, which decouples analgesic efficacy from many classic opioid adverse effects.

It exhibits high binding affinity and excellent selectivity for μOR over δOR, κOR and a broad panel of other GPCR targets. In preclinical studies, PZM21 delivers robust pain relief at equianalgesic doses with greatly reduced risks of respiratory depression, reward/reinforcing effects and constipation compared with morphine.

As a well-established tool compound in GPCR pharmacology, PZM21 serves as the parent scaffold for next-generation biased μOR agonists including FH210 and other analogues for safer analgesic discovery.
