Beraprost, mostly marketed as beraprost sodium, is a chemically stable, orally active prostacyclin (PGI₂) analogue discovered and developed by Toray Industries, Japan.
Basic Information
- Chemical Name: (±)-(1R,2R,3aS,8bS)-2-Hydroxy-1-[(3S,4S)-3-hydroxy-4-methyl-1-octen-6-ynyl]-2,3,3a,8b-tetrahydro-1H-cyclopenta[b]benzofuran-5-butanoic acid; Sodium salt
- CAS No.: 88430-50-6 (Beraprost); 157339-82-3 (Beraprost Sodium)
- Molecular Formula: C₂₄H₃₀O₅ (free acid); C₂₄H₂₉NaO₅ (sodium salt)
- Molecular Weight: 398.49 (free acid); 420.47 (sodium salt)
- Appearance: White or off‑white crystalline powder
- Originator: Toray Industries (Japan)
- Dosage form: Immediate‑release tablet / modified‑release tablet
Mechanism of Action
Beraprost binds to prostacyclin receptors on platelets and vascular smooth muscle cells, activating adenylate cyclase and raising intracellular cAMP. It inhibits calcium influx and thromboxane A₂ generation, producing three core effects: vasodilation, inhibition of platelet aggregation/adhesion, and endothelial cytoprotection. It also suppresses vascular smooth muscle proliferation and improves microcirculation.
Indications
- Chronic arterial occlusive disease: to relieve ulcers, intermittent claudication, resting pain and cold sensation in extremities
- Pulmonary arterial hypertension (PAH): improves exercise capacity and haemodynamics
- Off‑label / investigational uses: diabetic nephropathy, chronic kidney disease, peripheral microcirculation disorder
Key Pharmacological Features
- Route: Oral, active (unlike epoprostenol which is unstable and for infusion only)
- Typical clinical dose: 20–40 μg, three times daily after meals (IR formulation)
- Main adverse reactions: Flushing, headache, diarrhoea, hypotension, jaw pain (related to vasodilation)
- Caution: Risk of bleeding; use carefully together with anticoagulants, antiplatelet agents
Regulatory Status
Marketed in Japan, China and several Asian countries for peripheral arterial occlusive disease and PAH. It holds orphan‑drug designation for pulmonary arterial hypertension in the EU.
