Mechanism of Action

Elexacaftor is a next‑generation CFTR corrector. It binds to a distinct site on mutant CFTR protein. Combined with tezacaftor, it produces additive effects to improve protein folding, intracellular processing and trafficking of defective CFTR (including F508del mutation), increasing mature CFTR protein delivered to epithelial cell surface. It must be used together with ivacaftor (CFTR potentiator). The triple‑combination (elexacaftor + tezacaftor + ivacaftor, brand name Trikafta / Kaftrio) restores chloride‑channel activity, reduces viscous mucus and improves lung function in cystic‑fibrosis patientsDailyMed.

Indications

Elexacaftor is not for monotherapy, only for triple‑combination therapy with tezacaftor and ivacaftor: For the treatment of cystic fibrosis (CF) in patients aged 2 years and older with clinical diagnosis of cystic fibrosis, carrying at least one responsive CFTR gene variant (including the F508del mutation)DailyMed.
