Code: AZD7986, INS1007 Brensocatib is a first-in-class, oral, selective, competitive and reversible small-molecule inhibitor of dipeptidyl peptidase 1 (DPP‑1 / Cathepsin C). Originally discovered by AstraZeneca and licensed to Insmed, it is a key investigational molecule targeting neutrophil serine proteases (NSPs: neutrophil elastase, proteinase 3, cathepsin G).
DPP‑1 mediates the activation of NSPs during neutrophil maturation. Over‑activated NSPs drive persistent neutrophilic airway inflammation, mucus overproduction and progressive tissue damage in multiple inflammatory disorders. Brensocatib blocks this upstream activation pathway to reduce protease‑mediated injury.
The molecule has been approved in the US, EU and Japan for non‑cystic fibrosis bronchiectasis (NCFB) in patients aged ≥12 years with recurrent exacerbations. It is also under clinical evaluation for other neutrophil‑mediated inflammatory diseases including hidradenitis suppurativa and chronic rhinosinusitis without nasal polyps.
