Nerandomilast is an oral, preferential phosphodiesterase 4B (PDE4B) small-molecule inhibitor developed by Boehringer Ingelheim. It exhibits high selectivity for PDE4B over other PDE4 isoforms (PDE4A/C/D), which helps reduce the adverse effects commonly seen with non-selective PDE4 inhibitors.
PDE4B plays a central role in driving inflammation and fibroblast activation in lung tissue. Nerandomilast elevates intracellular cAMP levels, delivering dual anti‑inflammatory and anti‑fibrotic effects. It has been approved for adult patients with idiopathic pulmonary fibrosis (IPF) and progressive pulmonary fibrosis (PPF). Its pivotal FIBRONEER clinical program demonstrated that it slows the decline of forced vital capacity (FVC) and delays disease progression.
This compound is also under further investigation for other fibrotic and inflammatory disorders.
